An Aging Life Care Professional is there to assist individuals, families and other caregivers to navigate the complexity and challenges of eldercare. Through a comprehensive assessment with the client, family members and other caregivers, the Aging Life Care Professional develops a Plan of Care. The Plan of Care is specific to each clients needs and can include, arranging and coordination of care, attending medical appointments, providing advocacy within the healthcare system, oversee chronic disease management, provide medication management, act as a local point of contact for overall observation and be available in case of an emergency with your loved one.
